Anyone know of a cable that has 2 3.5mm mono male connectors to female 1/8th stereo?

I want to be able to connect my headphones to modules like the Intelligel Mixup or Doepfler A-138S mini mixer. Could also be a 1/4" female stereo connector. Either one would be fine. Thanks in advance.
